Medium (known as Regular in Guitar Hero Live) is the middle difficulty level in Guitar Hero (hence the name). This difficulty level uses only the GRYB fret buttons, ignoring the orange fret with few exceptions. In Guitar Hero Live, all fret six buttons are used (plus open strums) but there are less notes than Advanced difficulty.

Compared to Easy and Hard[]

Medium uses one more fret button than Easy, which uses only uses the GRY fret buttons (or one row of frets in Guitar Hero Live); and one less than hard, which uses all five buttons. It also has more chords and HOPO notes. It even has a few split chords in the more difficult songs.

Medium is much less difficult than Hard, mainly because the orange fret button is introduced at Hard difficulty (with few song exceptions). Also, Hard has three-note chords and more split chords. Medium also moves slightly faster than Easy, and with more notes. Some people find it easier to strum long strings of a note or a chord by simply strumming to the correct beat, whereas in Easy one must follow a restricted note pattern. This is true because eighth notes are introduced on this level, whereas other notes are used on Easy. This is why some people say it is less awkward to play Medium than Easy. Some might say that Medium is more "free" than Easy for those reasons. Also, while easy has a few hammer-ons, if any, per song, Medium really introduces them, with more complex songs having main riffs that involve hammer-ons and pull-offs.

In Guitar Hero Live, all six B1W1B2W2B3W3 fret buttons on the Guitar Hero Live Wireless Guitar Controller are used and open notes occasionally appear, however there are less notes than Advanced difficulty. There will be more more chords on Regular than Casual difficulty and introduces barre chords that Casual don't have, but just like Easy, all chords appear on Regular will either be on a single row or be barre chords (but no 3-note chords of any kind). Advanced difficulty will have chords in different columns on both rows.
